Our patient portal is a convenient resource for you. You will be pleased that you have chosen the Patient Portal to communicate in a secure and confidential manner with our healthcare team. Taking just a few minutes to register will give you access to valuable information and services provided in a secure and confidential manner. If you are not registered for the portal, contact the office for more information. Once registered and logged on you will be able to:  Ask a Billing/ Insurance Question  Request well appointments  Request prescription renewals  Request school forms and immunization records  Review Clinical Summaries from each office visit  Complete and update medical forms  Update your profile and contact information  Review and pay billing statements For urgent medical matters or in case of a medical emergency, call 911. Please list the forms you would like to receive and if you would like the forms to be mailed to you or if you would like to pick them up in the office. An email will be sent to you once the forms are complete. There is a $5 charge for all forms requests. Appointment Request For appointments request, if the office is able to meet your appointment request an appointment confirmation email will be sent. If the office is unable to meet the appointment request, you will receive a phone call to schedule your appointment. A voicemail will be left if no one answers.
